It insinuates the pain isn't real but, "in your head". I have a rare genetic pain disorder. I had to deal with doctors repeatedly insinuating or just stating, the pain wasn't real but, "in my head". Their ego's preferred to believe that if they can't diagnose it it isn't real. Finally specialist diagnosed Dercum's Disease. One of the most painful conditions there is.
