Re:Wikiprojects
Yes, usually people self-proclaim affiliation to wikiprojects. There may be projects where present members scrutinize a new applicant. But I have never come across such a project.
Yes, if you have expanded an article substantially, you can remove the stub template. Still better, after removing teh stub template, leave a note in its talk page that you have removed the stub template.
Other than discussing in the article talk page, related project's talk page is a very useful place to talk about an article's improvement. For example, Wikipedia talk:Noticeboard for India-related topics is an appropriate place to talk about articles related to India.--Dwaipayan (talk) 10:42, 21 April 2008 (UTC)
Wikiprojects:
People just go and add themselves to the project. Its like volunteering. We can call it self proclaimation....:-) It does not have much of a significance in what you can or can not do. Its basically an informal head count.
Stubs:
If you think that you have made a stub into a reasonably complete article, then you can. Its best to leave it in place and let someone else remove it. Doesn't really matter if you remove it either.
Rating:
You can go to other related pages and get rating templates, on the discussion pages. Just copy them, edit them as you may need and rate the page. If others agree, it won't be messed with, or else, someone else will change the levels (Quality/importance) or write on the discussion page requesting the change. Since you are a doctor, you may get the last say in case of medical articles, if you can justify it.

Hi. WikiProjects are only allowed to assign article quality ratings up to B. (Some projects have a special A rating, but the Medicine project does not.) The GA rating has a special process of its own. If you think a page meets the GA criteria, you can nominate it at WP:GAN. The instructions there will tell you how the GA process works.
